Health tips for winters

What to do for a healthier winter season!


A. Clothing: Keep yourself well-covered. Layered clothing is preferable as it remains light yet traps  warm air in between layers of clothes. The thinnest and lighter clothes should be worn inside and thicker ones outside.  In winters our body diverts blood from cold peripheral parts to vital internal organs. Wearing gloves or mittens, socks and cap to cover exposed areas helps body in maintaining body temperature. It is especially important in people who may have impaired sensations due to damage to nerves as they may not feel the cold or soreness and may suffer greater degree of damage. In times of high precipitation like rainfall or snowfall, one should wear outerwear that is waterproof and not let the clothes be damp.

B. Maintaining warm ambiance: Elderly and young children should avoid venturing outdoors in extremes of cold. It is preferable to go for a walk when the dew has dried. Closed doors and windows prevent cold drought of air from cooling the inside temperature. Curtains not only stop airflow from in between the jamb and doors but also create another layer of partition and prevent the air circulating in room to come in direct contact with cold window panes.
Rooms can be heated by a well ventilated fire-place with its dedicated chimney to reduce air pollution. Care should be taken to use only dry wood or coal as damp fuel causes more pollution and less heat. The room where fire is being burned should be well ventilated to prevent accumulation of gases like carbon dioxide (CO2) and carbon monoxide (CO). CO is poisonous and is colorless and odorless. It can lead to symptoms of headache, dizziness, nausea, vomiting etc.
Three types of heaters are available like hot air blowers, radiation heaters like halogen or carbon rod heaters and oil heaters. The air in winters is dry because cold air can hold less moisture. When this cold air is heated, though it becomes warm and rarefied, it remains dry. To reduce dryness caused by heaters, a container with wide mouth filled with water can be kept in room. Steam also humidifies the air and keeps the room warm. Oil heaters cause less drying up but are expensive.
Over heating the room should be avoided not only to conserve energy and reduce pollution but also to avoid drying up of mucus layer of our respiratory tract. Drying up of secretions and cracking up of the respiratory layer and skin allows the bacteria and viruses present in the air and on surface to enter the body and cause infection.

C. Maintaining Hygiene:  In winters, poor ventilation, huddling together, lack of sunshine and dampness favours microbes like flu viruses to persist, grow and get transferred from one to another. That is why winters are also called Flu season. Dampness and cold climates also help in growth of indoor mold. So, it is important to maintain hygiene in winters by frequent hand washing, covering coughs and sneezes and avoiding touching mouth and nose repeatedly. It is essential to take bath regularly and change clothes. Water should be warm and not too hot. Hot showers should be reduced to less than 10-15 minutes so that the oil layer on skin that prevents loss of moisture is not lost. Using moisturizers and oils helps in maintaining healthy skin.

D. Diet: It is important to maintain hydration in winters to prevent drying up of our respiratory lining and skin. Excess of drinks like coffee and tea that cause repeated urination should be avoided. Home made soups are better than ready made premixes as they contain high sodium content that can raise the blood pressure. Risks associated with alcohol intake outweigh any benefits. The warmth generated by alcohol is quickly dissipated as it causes widening of blood vessels in our skin.
It is best to consume seasonal fruits and vegetables to maintain nutrition. Dry fruits in moderate quantities are also beneficial. While energy requirement may be higher in winters to maintain body temperature, too much of calorie dense foods like sugar and chocolates should be avoided.

E. Sleep & Exercise : It is essential to take adequate quantity of sleep that usually is 7-9 hrs per day. However, longer nights and lack of sunshine can lead to season associated mood disorder. Therefore, exposing oneself to whatever sunshine is available prevents development of winter blues. Exercise also helps in keeping one fit and elevating mood. While heavy outdoor exercise should be avoided to prevent too much strain on heart, light indoor exercises can be helpful in maintaining activity level.

F. Medication and Vaccination: Those who have chronic illness must take medicines regularly. Some patients of asthma may need to increase the dose of their medication besides avoiding direct exposure to cold air. They should also take measures to check any growth of mold. Presence of contaminated heavy upholstery, especially if it gets wet can be harmful. Vacuum cleaning can be helpful in keeping surrounding clean and reducing allergen load. Elderly people and patients with chronic ailments must take their scheduled influenza and pneumococcal vaccines before the onset of winters.

Keeping oneself updated with the information about weather as available through various media helps us being prepared to enjoy the winters most. The requirements for different individuals vary and we should take measures that suit us best, however the aim should be to make oneself comfortable and avoid overindulgence.

प्लास्टिक काल Plastic era

प्लास्टिक 

कहते हैं सभी कि सत्य अटल है
फिर क्यों ये समस्या इतनी जटिल है
अपने जीवन को सुविधापूर्ण बनाने में तो कोई हर्ज़ नहीं
पर क्या इस सुख की इच्छा से बढ़कर कोई फ़र्ज़ नहीं?

अपनी अभिलाषायों को अर्जित करना
ही होती है विकास की पहचान
पर अपने इस अभियान में
बहुत कुछ भूल जाता है मनुष्य अनभिज्ञ, नादान!

जीवन को सरल करने को ही खोजा प्लास्टिक
और आज हमारी ज़िन्दगी ही हो गयी प्लास्टिक,
जन्म के साथ ही जीवन में प्लास्टिक अवतरित हो जाता है,
आजकल तो गर्भनाल को भी प्लास्टिक से बाँधा जाता है,
प्लास्टिक की नैप्पी, प्लास्टिक का बिछौना,
प्लास्टिक की बोतल, प्लास्टिक का खिलौना,
किताब रखने का बस्ता भी प्लास्टिक
और उसे पढ़ने का चश्मा भी प्लास्टिक
बस का पास भी प्लास्टिक
रजाई की कपास भी प्लास्टिक
शीशी दवाई की प्लास्टिक
पर्स में कमाई भी प्लास्टिक
प्लास्टिक का कंप्यूटर, प्लास्टिक की कलम
प्लास्टिक की नलियों के बीच निकला दम
प्लास्टिक के फ्रेम में टंगी फोटो पर प्लास्टिक का हार,
तर्पण में प्लास्टिक की कैन से बहे गंगाजल की धार!

हमने तो प्लास्टिक का अंतरिक्ष यान तक बना डाला
ये सोचकर कि प्लास्टिक हमें दूर तक ले जायेगा,
पर भूल गए कि हम न रहेंगे, तुम न रहोगे
सिर्फ प्लास्टिक ही रह जायेगा!
पहुंचना चाहते हैं स्वर्ग बनाकर प्लास्टिक की सीढ़ी को
नहीं सोचा क्या मूहँ दिखाएंगे अपनी भावी पीढ़ी को

मनुष्य नश्वर है, अल्पकालिक है
और प्लास्टिक मानों अमर, सदैव
जब पृथ्वी पर ही नहीं, सभी गृहों पर, अंतरिक्ष में
रह जायेगा सिर्फ प्लास्टिक दैव
तो क्या वो प्लास्टिक काल कहलायेगा?
पर वो देखने को कौन रह जायेगा!

तो सोचें क्या होगा समय के उस पार
जब प्लास्टिक काल में मनुष्य पहुंचेगा यम के द्वार
चित्रगुप्त बैठे होंगे, मनुष्य करेगा अपनी गुहार
पर चित्रगुप्त टस से मस न होंगे
न चेहरे पर कोई विकार
करेगा पूजा जलाकर प्लास्टिक का दिया वो
जब मनुष्य की जाएँगी सारी विनती बेकार
फिर पिघलेंगे चित्रगुप्त, मन से नहीं तन से,
तब समझेगा कि चित्रगुप्त तो हैं प्लास्टिक की एक प्रकार
प्लास्टिक के यम, प्लास्टिक का पौंड्रक
फिर बैकुंठ पहुँच करेगा मनुहार
पर वहां भी प्लास्टिक की लक्ष्मी, प्लास्टिक के शेषनाग
यहाँ तक कि नारायण भी प्लास्टिक
तब वहां से भागेगा मनुष्य करके हाहाकार
फिर नारद उसको समझायेंगे
सुनकर उसकी चीख पुकार
“तुमने प्लास्टिक की दुनिया बसाई
प्लास्टिक का सारा संसार,
कृत्रिम या नकली ही प्लास्टिक का अर्थ है
तुम्हारा यूं लोक-परलोक फिरना व्यर्थ है
जो तुमने देखा, सब नकली था
अब तो सुधर जायो यार
त्याग दो प्लास्टिक
स्वयं करो अपना उद्धार!


आज सरल नहीं है
खोजना प्लास्टिक का विकल्प
पर करना होगा हम सबको
एक दृढ़ संकल्प,
क्या कागज़ का उपयोग प्लास्टिक से बेहतर है?
जलवायु परिवर्तन के समय कठिन देना इसका उत्तर है!
प्लास्टिक न हो जाये सागर में जीवों से ज़्यादा
इसके लिए खुद से ही करना होगा पक्का वादा!
शायद सुविधा को ताक पे रखना होगा
मानव को अग्नि की पैनी धार पे चलना होगा
जब अपनी ही चमड़ी से
हम जूते नहीं बनाते,
तो वर्तमान में सुविधा के लिए
क्यों भविष्य को नोच-खाते?

मनुष्य ही मनुष्य का शत्रु है
तो फिर क्या है उपाय ?
मानव ही है मानव का मित्र भी
ये भी न भूला जाय!
हम ही निकालेंगे इसका भी हल
फिर ये समस्या भी लगेगी सरल,
तब तक
करो कटौती, पुन: प्रयोग
पुन: चक्रित भी कर डालो
जब तक निकले न बड़ा हल
छोटी युक्तियाँ निकाल डालो,!
इस उत्कर्ष के दौर में
न हताश हो, न निराश हो
न बिखर अभी, न उदास हो
अब हो गया है शामिल तू
आंदोलन इस ख़ास में
अब तो बस हर्ष हो,
अब तो बस उल्लास हो!

यार! एक बार.. आ जाओ,

ये आवाज़ मेरी नहीं,
उन लम्हों की है जो कहानी बन गए,
सुनलो वो कहानी,
या अपनी ज़ुबानी
फिर से सुना जाओ,
यार! एक बार.. आ जाओ,
और कैसे बुलाऊँ बता जाओ!

अब भी खुदा है
इन दीवारों पर तुम्हारा नाम
पर सूरत धुंधला गयी है
एक बार... झलक दिखला जाओ,
यार! एक बार.. आ जाओ,
और कैसे बुलाऊँ बता जाओ!

यूँ तो इस मशीनी ज़िन्दगी में
पहले भी बहुत था
और बाद में भी बहुत है,
पर उस साथ गुज़ारे वक़्त
की बात ही कुछ और है,
गर नहीं है... तो मुझे ही समझा जाओ,
यार! एक बार.. आ जाओ,
और कैसे बुलाऊँ बता जाओ!

होंगे कुछ शिकवे भी
जो यहीं जन्मे थे

Munger - Bihar

Bird Flu...yet again!!
After 2009, due to pandemic of Swine Flu, I thought my services as a respiratory physician familiar with influenza management, had become redundant. This time it was to be Munger in Bihar. I was very neutral to the news that I would need to visit Munger to set things in order as I did not see it coming in way of anything important. Though I was skeptical about the living conditions there, but I knew it would not be much different from Purulia or Murshidabad.
As I reached airport to board my flight to Patna, an excitement started to build up. Excitement??? for Bihar??? Sounds very strange. But I was not the only one who had excitement for Bihar trip!!
I have spent my adolescence in a boys hostel. Though our hostel had students from various places, many from abroad but there was a predominance of Biharis and 'Chinkis' as we used to call all guys from north-east. Eventually there were two big groups, that of Biharis and Chinkis. In my heart, I was neutral but because of my appearance many people used to think that I must be from Bihar, and nobody ever took me to be a Manipuri, obviously! When I became little senior, I had picked up the Bihari accent and diction nuances. Whenever a new student from Bihar would join our hostel, I would speak to him as if I am a Bihari so that he could identify with me and would try to make him comfortable. I could rattle the names of various districts.. Darbhanga, Madhubani, Chhapra, Betiah, Muzzafarpur, Dhanbad (now in Jharkhand) and so on.... but when somebody asked my place, I would always say Patna, as I would be caught soon if I said about a Maithili or Magahi speaking district. I was not even sure that which district fell in which region but it was comparatively easier to mimic Bhojpuri. So often I would talk about Gandhi Maidan or Exhibition road to sound familiar with Patna as that were the names I had heard from my friends. So, I have virtually lived in Bihar for 30 years with some of my closest friends without ever having been there.
I almost had goose-pimples on landing at Patna. I was euphoric seeing Gandhi Maidan from the window of my room. I could have never imagined that I would be so excited to visit Bihar. Though my friend couldn't come to meet me because of his distant posting but he virtually hosted me. On his suggestion, I walked down to a place known for its Litti Chokha. I have had the dish few times in past but had never liked it so though I went but was reluctant to try it. But then, I tried! Litti was served as a freshly grilled ball of dough with stuffed Sattu, which is powdered roasted gram with some condiments. After barbecuing the litti balls they dipped it in 'Desi ghee' and served with Chokha - a potato preparation with light gravy. It reminded me of the fresh doughnuts that I had at Pier 36 in San Fransisco during my US visit. Delight was same! Same circuits of pleasure lighted up in my brain. The tang of mustard oil was perfectly balanced. I ordered another pair!

To complete the experience, I bought a Kurta Pyjama in the colours that I have never worn but a Bihari person is used to alongwith a 'Bundi', the sleeveless jacket of Khadi, usually worn by politicians.
Munger was quite what I had expected. My lodge was in a market next to bus stand so it was very noisy all the time. Honking of horns, noise of trollers and loud music would begin at 4 am and continue till 10 at night. Everybody seemed fond of 80s-90s music played loudly.
Munger is an important place in history and has been continuously inhabited ever since 11th-12th century. It is in the region of 'Anga', the territory of Karna, as per Mahabharata. There is a Shakti Peeth, called Chandika Sthan located in a cave called Karna ki karahai (inverted). The sanctum sanctorum has a large eye of silver with cornea of gold with a diamond (supposedly) at Pupil that is the object of veneration here. There is another place called Sita Kund, that has 5 kunds, one has hot water, called Sita Kund and other 4 have called water named after Lord Ram and his 3 brothers. The kunds get flooded when Ganga river gets flooded and therefore are connected to the river water but still one kund has warm water.
The art of making perfect Litti - all the steps can be seen. Making the dough ball, stuffing Sattu, roasting it on a grill and then dipping in Ghee before serving!
Ganga, here is called, 'Uttarvahini' as it meanders towards north against its principal direction of south-east. Munger is located in the niche of land so formed. One gets this beautiful view from 'Pir Pahari' that appeared like the highest point of Munger. There are 'Mazaars' at the top, two of them are said to be of sons of Mir Kasim and other one of Shah Suja, brother of Aurangzeb, who took refuge here. One also sees a tunnel that was supposedly leading to inside the Munger Fort but now is blocked due to roof collapse.

The gate of Munger Fort is unmistakable and is located right next to the bus stand. There is a large area inside the fort that has many administrative buildings and leads to the banks of river Ganga, the most popular spot is known as Kashtharni Ghat but is not well maintained, as the river recedes. The Munger museum, that has artifacts of various ages excavated from here and surrounding areas, is also within the compound. However, the most remarkable is 'the Bihar School of Yoga. It is a place of international repute, dedicated to teaching the benefits of Yoga, without much ado. In fact, they refrain from entertaining anybody, who seems like a non-serious visitor. There is no pomp.
Munger has lot of gun shops, probably factories for desi katta also but I did not explore that side of the district!
On my way back in train, there were lot of vendors offering kulhad chai, jhaal-moori but my pick was chai lemon. Chai lemon  was a new experience! It was not the lemon tea that we usually drink. The guy actually squeezed a lemon into the black tea that he was carrying and to my surprise sprinkled some powder in it. It tasted like a new hot mocktail, something like kala-khatta! I asked him what all was there in the powder and he rattled 'kala namak, lavan bhaskar............& so on'! It caressed my palate for quite some time!
Another palpable thing was during my field visits, I noticed that contrary to my previous experiences ODF campaign has in fact made changes and though still lot remains desired but human excreta was noticeably absent from most places.
I came back with a much better opinion about both urban and rural Bihar.

धुएँ की तिज़ारत

एक ज़िन्दगी रोज़ नए मायने तय कर रही है
और एक आज भी उस अधूरी करवट में रुकी है
एक निगाह क्षितिज पर गड़ी है
पर एक मायूसी से परस्पर झुकी है
जिस दरख़्त को बड़ी शिद्दत से सींचा था
जिसकी हर टहनी को बाँहों में भरा था
हर टूटते पत्ते को दांतों से भींचा था
जिसके ज़हर को भी गले उतारा था
हर हाल में पनपेगा वो
बस एक यही वादा था
उस पेड़ को आग लगा आया हूँ

मेरी सब नादानियाँ जो सह गया
भला एक नेकी से कैसे बह गया?
जिसने उसे खोखला किया
वो फरेब की नहीं थी,  वो झूठ की भी नहीं थी
वो दीमक मुसाबक़त की थी
जिससे दर्द की रेत रिसने लगी
उस दरख़्त को आग लगा आया हूँ

दूर हूँ, दीखता नहीं,
पर  अब यक़ीनन दर्द धुआँ हो गया है
जो गले के ज़हर को उबालता है
मेरी हर ख़ुशी को अधूरा करता है
खोखलेपन का इलाज
शोलों से भर, कर आया हूँ
चारागर हूँ, दवा कर आया हूँ
अब न दरख़्त है, न खोखलापन
और न आग
बस धुआँ है
और धुएँ में बसा दर्द
एक एहसास ने सब बेच दिया
और सब के बदले
मैं ये धुआँ  खरीद लाया हूँ
आखिर मैं उस दरख़्त को आग लगा आया हूँ

उसके सुलगते ठूंठ को
कोई पानी से बुझा दो
कभी पानी से जिसको खूब पाला था
अब भला कैसे उसकी प्यास बुझाऊँ
मैं तो उस दरख़्त को आग लगा आया हूँ!!
